Worms after a water change?

Ok so I just got done with water changes in my tanks. I was looking in one of my 2" gallon tanks where I have some fry and saw some worms swimming around. At first I thought my eyes were playing tricks in me but they were definitely some type of worm. They were swimming. Then the fry started eating them. Any thoughts? I only feed them pellets so I have no clue how they would have gotten in there.

Planaria  usually occur when over feeding, or very dirty gravel.  They are not harmful to your fish but a sign of poor conditions really. Clean that gravel real well.  Post a picture but it's probably what it is.

I am pretty good about my water changes. I usually so not let it go more that two weeks. I could be over feeding them. I only saw a few of them and what I did see the fish were eating so I am pretty sure they are all gone. I will have to keep my eye on it. Thanks.

So I have to bring this one back up. I recently bought a 195 setup from Great Lakes. When I was filling it using the hose I do my water changes with (the clear vacuum/filling hose) I noticed these worms again. I figured that they are just somehow living in the tube. (It's a brand new setup so where else could they be from) but this leads to another question. I recently moved some fish out of a 20 gallon tank and it's been running empty for a few days. I just looked in there and saw what I can only describe as snails? Look at the pictures and let me know if anyone else has seen this before. I have no idea how something like this got in there. And could they be related to the worms I am seeing?

Malaysian trumpet snails?  I get those little white looking worm things when I overfeed.

It's just weird that the worms were there when I filled up my brand new tank. They must be living inside my filling hose somehow. Any idea how the snails got in there?
